To submit an application for PCA’s Triple-Impact Competitor® Scholarship Program Presented by Deloitte and Thrive Foundation for Youth, read our rules to ensure you are eligible and then complete the entire form below. (You cannot save your work in this form, and it must be completed before submitting, so consider writing your longer answers in a desktop application and then pasting into our form.)

Who is Eligible
Any student-athlete who is currently a junior (class of 2013) with a grade point average of 2.5 or above in the following geographic regions:
Chicago Area: Counties of Cook, DuPage, Kane, Kendall, Lake, McHenry and Will.
Houston Area: Counties of Brazoria, Chambers, Fort Bend, Galveston, Harris, Liberty, Montgomery and Waller.
New York City Tri-State Area: (NY) Counties of Bronx, Kings, Nassau, New York, Putnam, Queens, Richmond, Rockland, Suffolk, Westchester. (NJ) Counties of Bergen, Hudson, and Passaic. (CT) Counties of Fairfield County, New Haven County, Litchfield.
San Francisco Bay Area/Sacramento Area: Counties of Alameda, Contra Costa, El Dorado, Marin, Placer, Sacramento, San Francisco, San Mateo, Santa Clara, and Yolo.
Washington, DC Area: (VA) Arlington, Alexandria, Fairfax, Loudon, Prince William, and Stafford counties and Cities of Alexandria, Fairfax and Manassas. (MD) Counties of Anne Arundel, Calvert, Charles, Frederick, Montgomery and Prince George's.
Application Period
The application period for 2012 awards opens on January 1, 2012 and closes on April 30, 2012 at 11:59pm Pacific Standard Time. Applications submitted after that time will not be considered.
Criteria
A Triple-Impact Competitor makes positive contributions on three levels:

Makes oneself better (Personal Mastery);

Makes teammates better (Leadership); and
- Makes the sport better (Honoring the Game).
Completed Applications
A completed application comprises:

Student-Athlete Information Form and Personal Statement
-
A letter of recommendation from a school administrator (athletic director, teacher, counselor, etc.)

A letter of recommendation from a coach

At least one Testimonial (and up to three) to be submitted by individuals who are familiar with the student-athlete and able to speak to his/her embodiment of the Triple-Impact Competitor principles.
Selection and Notification
The Triple-Impact Competitor Scholarship Program Selection Committee will select award recipients. The number and dollar amount of scholarships will vary by regions. All scholarship finalists will be notified via email in August, 2012. Scholarship winners will be announced at awards events in Autumn, 2012.
